The Belmont Report’s distinction between research and the practice of accepted therapy has led various authors to suggest that these purportedly distinct activities should be governed by different ethical principles. We consider some of the ethical consequences of attempts to separate the two and conclude that separation fails along ontological, ethical, and epistemological dimensions. Clinical practice and clinical research, as with yin and yang, can be thought of as complementary forces interacting to form a dynamic system in which the whole (...) exceeds the sum of its parts. Just as effective clinical practice cannot exist without clinical research, meaningful clinical research requires the context of clinical practice. We defend this thesis by triangulation, that is, by outlining how multiple investigators have reached this conclusion on the basis of varied theoretical and applied approaches. More confidence can be placed in a result if different methods/viewpoints have led to that result. (shrink)

In the modern world, provincialism in reflective thinking is dangerous, possibly tragic. If philosophy is to fulfill one of its main functions—that of guiding the leaders of mankind toward a better world—its perspective must become worldwide and comprehensive in fact as well as in theory. This, the motivating theme of the Second East-West Philosophers' Conference held at the University of Hawaii in the summer of 1949, is likewise the theme of this volume, the complete report of that Conference. The (...) goal of the eminent philosophers participating was the discovery of avenues of progress toward a synthesis of Oriental and Occidental thought. They attempted to reach tentative conclusions in the fields of methodology, metaphysics, and ethical theory. Conference activities consisted of meetings, lectures, discussions, undergraduate courses, and graduate seminars. As far as possible, the proceedings were limited to the study of East-West philosophy; extensive attention was not given to the strictly religious aspects of any philosophical tradition. Participants in the Conference were convinced that philosophy is now entering upon a new stage of development which will be characterized by trans-cultural cooperation and world perspective. This development can be of tremendous significance in world thought and eventually in the lives and actions of the people of the world. Twenty-three of the formal papers presented at the Conference comprise the substance of this book; a significant proposal for synthesis is stated or implied in almost every one. The purpose of this paper is to present the results of a natural experiment involving academic cheating by university students. We explore the relationship of moral judgment to actual behavior, as well as the relationship between the honesty of students self-reports and the extent of cheating. We were able to determine the extent to which students actually cheated on the take-home portion of an accounting exam. The take-home problem was not assigned with the intent of inducing cheating among students. However, (...) the high rate of observed cheating prompted the instructor to return to class and ask the students to provide information on their motivation. The students' responses are the data analyzed in this natural experiment. We found that in a simple regression the relationship between moral judgment scores and cheating behavior was insignificant. However, when we tested whether including Utilizer scores affected the relationship of cheating and moral judgment we found that Utilizer affected the relationship significantly. Finally, we found that moral judgment and honesty were not related, but higher levels of cheating behavior related to less honesty. (shrink)

PURPOSE: There is an increasing demand for researchers to provide research results to participants. Our aim was to define an appropriate process for this, based on needs and attitudes of participants. METHODS: A multicenter survey in five sites in the United States and Canada was offered to parents of children with cancer and adolescents with cancer. Respondents indicated their preferred mode of communication of research results with respect to implications; timing, provider, and content of the results; reasons for and against (...) providing results; and barriers to providing results. RESULTS: Four hundred nine parents (including 19 of deceased children) and 86 adolescents responded. Most parents (n = 385; 94.2%) felt that they had a strong right to research results. For positive results, most wanted a letter or e-mail summary (n = 238; 58.2%) or a phone call followed by a letter (n = 100; 24.4%). If the results were negative, phone call (n = 136; 33.3%) or personal visits (n = 150; 36.7%) were preferred. Parents wanted the summary to include long-term sequelae and suggestions for participants (n = 341; 83.4%), effect on future treatments (n = 341; 83.4%), and subsequent research steps (n = 284; 69.5%). Understanding the researcher was a main concern about receiving results (n = 145; 35.5%). Parents felt that results provide information to support quality of life (n = 315; 77%) and raise public awareness of research (n = 282; 68.9%). Adolescents identified similar preferences. CONCLUSION: Parents of children with cancer and adolescents with cancer feel strongly that they have a right to be offered research results and have specific preferences of how and what information should be communicated. Background: Low rates of participation of adolescents and young adults (AYAs) in clinical oncology trials may contribute to poorer outcomes. Factors that influence the decision of AYAs to participate in health research and whether these factors are different from those that affect the participation of parents of children with cancer. Methods: This is a secondary analysis of data from validated questionnaires provided to adolescents (>12 years old) diagnosed with cancer and parents of children with cancer at 3 sites in Canada (...) (Halifax, Vancouver, and Montreal) and 2 in the United States (Atlanta, GA, and Memphis, TN). Respondents reported their own research participation and cited factors that would influence their own decision to participate in, or to provide parental authorization for their child to participate in health research. Results: Completed questionnaire rates for AYAs and parents were 86 (46.5%) of 185 and 409 (65.2%) of 627, respectively. AYAs (n = 86 [67%]) and parents (n = 409 [85%]) cited that they would participate in research because it would help others. AYAs perceived pressure by their family and friends (16%) and their physician (19%). Having too much to think about at the time of accrual was an impediment to both groups (36% AYAs and 47% parents). The main deterrent for AYAs was that research would take up too much time (45%). Nonwhite parents (7 of 56 [12.5%]) were more apt to decline than white parents (12 of 32 [3.7%]; P < .01). Conclusions: AYAs identified time commitment and having too much to think about as significant impediments to research participation. Addressing these barriers by minimizing time requirements and further supporting decision-making may improve informed consent and impact on enrollment in trials. (shrink)

In Britain, policy‐makers tend to view racism as a social attitude rather than an institutional/structural phenomenon. Not until the publication of the MacPherson Report (1999) was the idea of ‘institutional racism’ officially recognised. According to Jules Holroyd, implicit bias as a concept can help us understand and combat the kind of unwitting prejudice the Macpherson report describes. This article explores whether implicit bias is indeed a viable framework for understanding institutional/structural racism. To do so, I bring together Charles Mills’ (...) notion of ‘global white ignorance’ and Lewis Gordon's interpretation of ‘bad faith’. Through Mills’ and Gordon's analyses, which together illuminate both the structural and psychic dimensions of racism I offer an account of the psychodynamics of racism far more consistent with our observations of how racism actually operates in Britain. Specifically, we see that institutional/structural racism is neither unconscious nor is it unmotivated as implicit bias would suggest. As such, I reject implicit bias as a useful or necessary explanatory framework for helping us understand institutional racism as a structural phenomenon. (shrink)
